Event Waiver of Liability
I, the undersigned, agree to hold harmless, PEER Group of Cincinnati, Inc., it’s Board of Directors, officers, organizers, DM’s and members; it’s affiliates and their members; other fellow participants; the owner or the tenant of the hosting facility, and it’s landlord, from any and all liability pursuant to any physical, emotional, psychological, or other injury which may occur during or as a result of my voluntary participation in this PEER Group of Cincinnati, Inc. sponsored event of xx/xx/xx - xx/xx/xx.
I, the undersigned, am at least 21 years of age or older, and am legally capable of participating in adult oriented activities and will provide legal proof of age at any time requested.
I, the undersigned, understand that adult oriented discussions, activities, and/or demonstrations may occur and I am not offended by such; understand that there may be nudity and I am not offended by such; state that my attendance and/or participation are totally voluntary and that I may leave at any time if I am or become uncomfortable with what I observe.
I, the undersigned, understand that the activities that may occur during this PEER Group Cincinnati, Inc. sponsored event may pose a physical and/or psychological risk for some individuals. It is my responsibility to make my own judgment as to my ability to participate safely in these activities. It is my responsibility to make known any medical, allergic, and/or psychological conditions that may have a bearing upon my participation in these activities, but at no time will it be the responsibility of PEER Group of Cincinnati, Inc., it’s Board of Directors, officers, organizers, DM’s, members; it’s affiliates and their members; other fellow participants; the owner or the tenant of the hosting facility and it’s landlord, to protect me from such perils.
What I observe here will be kept in strict confidence, and that I will do nothing to jeopardize the privacy and identity of the other attendees. I agree to not create any audio or video recordings, nor take any photographs, or record the activities of other attendees. I agree that any audio, video, photographs, or/and records produced at this event are the sole property of PEER Group of Cincinnati, Inc. regardless of their creator. If I am the creator of said materials then I agree to transfer all copyrights of these materials to the PEER Group of Cincinnati, Inc. immediately with my signature below.
I, the undersigned, agree to leave voluntarily and immediately, with the forfeiture of the admission donation, for just cause, at the request of a member of the Board of Directors or DM of PEER Group of Cincinnati, Inc., or their designated representative(s).
Just cause will include but not be limited to:
· Violation of the General rules of PEER Group of Cincinnati, Inc.; or disruptive and/or destructive behavior · Being under the influence or the use of, recreational drugs, alcohol, or illegal substances · Being in position of a firearm · The exchange of body fluids or sexual penetration (defined as the insertion, however slight, of any apparatus, instrument, or body part into the oral, vaginal or anal cavity of another). · Failure to respond to the safeword(s), safe signal, or instructions of any authorized DM, or failure to respect the limits of behavior set by other individuals for themselves.
I, the undersigned, realize that the above rules are necessary to insure that this PEER Group of Cincinnati, Inc. sponsored function remains within the legal statues of our area, and remains voluntarily safe, sane, and consensual for the enjoyment of all the attendees.
I the undersigned, have read, understand, and agree to follow all of the above statements, and PEER Group of Cincinnati, Inc. Event Space Rules dated 11/01/08 revision 26 on the reverse side. My signature also indicates that I am not here in my official capacity of a law enforcement officer, postal service agent, or governmental employee or agent, nor am I, a registered sexual predator, habitual sex offender, or sexual oriented offender.
